This form allows you to request and reserve the upstairs area of the clubhouse. Each group must submit this form once a year to reserve the area for their activity.
Using the Paper/Printed Form
- You can download and print the form or pick up a copy at the office.
- When you are done completing the form, you can scan the form and email it to the office, take a photo of the form and email it to the office, or mail or drop-off the completed paper copy to the office. Keep a copy just in case something happens.
Using the Fillable PDF
- The fillable form helps create entries that are easier to read, and helps clarify date and time field values.
- You can type directly into the PDF form on a computer or smart phone that has a PDF reader, such as Adobe Reader.
- If your event requires a setup of tables and chairs, you still need to draw the layout. In this case, print the form and draw the setup you want. Then you can submit the form as described in the Using the Paper/Printed Form section.
- If no setup is required, you can fill out the PDF, save it, and then email it to the ORO Office. If you prefer, you can print the saved PDF file, and then submit the form as described in the Using the Paper/Printed Form section.
- For duplicated fields, such as the Event Description that is on both page 1 and 2, you can copy the value from one field and paste it in the other.
Notes about the Fillable PDF
- On some systems, you can select values in fields, such as dates. When you tab into a field, a small box may be displayed with a down arrow that allows you to select the value.
- If you specify an invalid value, the system may display a blank in the field, or the system may display an error message.
- You can change a checkbox by clicking or tapping on the checkbox. Some systems allow you to tab to a checkbox and then press the space bar to check or uncheck the checkbox.
- If you mouseover a field, some systems display text to help you identify what to provide in that field.
Dates in the Fillable PDF
- Must be entered in the MM/DD/YY format. Some systems allow you to select a date value.
- Valid examples: 02/02/23, 02/02/2023, 2/2/23
- Invalid examples: 02-02-23, Feb 2 2023, 02 02 23
- Warning: An entry like 2/2 may work, but the year will be the current year, which may not be what you want since February 2, 2022 is in the past.
- If you specify an invalid value, the system may display a blank in the field, or the system may display an error message.
Times in the Fillable PDF
- Must be entered in the HH:MM format. Some systems allow you to select a time value.
- Valid examples: 10:30, 22:30, 09:00
- Invalid examples: 10, 10:, 10 PM, 10 30, 10.30.
- On some systems, 4:00 is invalid, but 04:00 is valid.
- To be clear about times, please use military time. In military time, 7 PM is 19:00.
- If you specify an invalid value, the system may display a blank in the field, or the system may display an error message.
